AMBS Health Insurance
Students registered for a minimum of six credit hours on the Elkhart campus are required to enroll in the accident and health insurance plan offered by American College Student Association (ACSA) unless they show proof of coverage with another provider AND submit a waiver form by the close of registration for additional courses and changes to grading plans.
Any student who is enrolled at least 6 credit hours on the Elkhart campus and who does not complete the waiver form online by the close of registration for additional courses and changes to grading plans (the drop/add period) will be enrolled in the Budget Plan and will be charged the appropriate premium for coverage for the student only. The fee is non-refundable. Higher coverage (Low Option or High Option Plans) and coverage for spouse and/or child(ren) is available for purchase at a higher premium (see below).

Waiver application
Waiver forms open approximately one week before classes begin. For the 2012-13 academic year, waivers must be completed by the following dates:
- Spring semester: 4 p.m. Monday, February 18, 2013
